Who Is Responsible for Your Information
Wealth Forms is an Altitude product. When your advisor sends you a form, your advisor's firm decides what to ask and what to do with your answers. Wealth Forms handles that information on the firm's behalf, as the firm's service provider.
What We Collect
- Your answers. Whatever the form asks for, which may include your name, contact details, Social Security number, employment, family and account information.
- How to reach you. The email address or phone number your advisor has on file, used to send your link and your Secure Form Code.
- Signature records. If you sign, the time, your IP address, your browser, how you verified yourself and a fingerprint of what you signed.
- Progress. When you opened the form and how far you got, so your advisor knows whether you need help.
How It's Used
Only to deliver the form, verify it's you, record your signature and return your answers to your advisor's firm. Wealth Forms doesn't sell your information or use your answers for advertising.
Who Can See It
Your advisor's firm. Altitude staff can access systems only to run and support the service. Service providers that help us run Wealth Forms, such as hosting, email and text message delivery, process information only to provide that service.
How It's Protected
Answers are encrypted in storage and every page uses HTTPS. Sensitive answers are cleared from the form once your advisor applies them to your record. See Security for details.
How Long It's Kept
Your advisor's firm keeps your records as long as it needs to, including to meet its regulatory record-keeping duties. Ask your advisor about their retention practices.
