Client forms that arrive already filled in.
Wealth Forms pulls what you already know from Altitude CRM. Clients confirm what's right, fix what changed and sign. You approve every change before it updates their record.
Got a form from your advisor? Here's what Wealth Forms is.
Annual Review Update
- Prefilled from Altitude CRM
- One-time code on every form
- E-signature with a full record
- Your logo and your domain
Clients Start With What You Already Know
Every field Altitude CRM has on file is filled in before the form goes out. Clients read, confirm and change only what's different. Most finish in a few minutes.
- Names, addresses, employment and contact details
- Household members, dependents and beneficiaries
- Sent under your firm's name, from your contact's record
You Approve Every Change
When a form comes back, you see each answer that changed, next to what was on file. Apply the changes and Altitude CRM updates. Nothing gets retyped, and nothing slips in without a look.
- Old and new values side by side
- One click to apply to the client's record
- Signed PDF kept with the contact
Works With the Forms You Already Use
Upload a custodian or vendor PDF and place fields right on the page. Clients fill it online, and you download it back exactly as the custodian expects it.
- Fillable PDF for clients who prefer paper
- Scanned copies read back into the same review
- Your own forms built in the form builder
Also in Every Plan
E-Signatures With Evidence
Clients consent first, then sign. Each signature records when, how and exactly what they signed.
Progress Tracking
See who opened their form, how far they got and who needs a reminder.
Your Brand
Import your logo, colors and fonts, and use your own domain for form links.
Repeating Sections
Dependents, beneficiaries, businesses and accounts repeat as often as a client needs.
Phone Friendly
Forms fit any screen. No account and no app for your clients.
Built Into Altitude
Send from a contact, track on their Forms tab and keep the signed copy on the record.
Made for Sensitive Client Data
Client forms carry Social Security numbers, account numbers and family details. Every part of Wealth Forms is built around keeping that safe.

A Code Before Every Form Opens
A one-time Secure Form Code by email or text. It expires in 10 minutes and works once.
Answers Encrypted
Form answers are encrypted in the database, and sensitive values are masked on screen.
Nothing Changes Without You
A submitted form never overwrites a client record on its own. An advisor applies it.
Sessions Stay With the Device
A forwarded link doesn't carry an open session along with it.
Tamper-Evident Signatures
Each signed form gets a SHA-256 fingerprint. Any later change breaks the match.
Sensitive Exports Logged
Exporting a Social Security number or other sensitive value to a PDF is recorded in the audit log.
Got a Form From Your Advisor?
Your financial advisor uses Wealth Forms to collect and update your information securely. It's safer than emailing documents back and forth.
Most of the form is already filled in from what your advisor has on file. Check each page, update anything that changed and sign at the end. No account or app needed.
Questions about the form or your accounts? Your advisor is the best person to ask.
How to Check an Email Is Real
- It names your advisor and their firm, and you were expecting to hear from them.
- It comes from an address ending in
wealthforms.com, or from your advisor's own domain. - The form asks for a one-time code, sent to you separately by email or text.
- Still unsure? Call your advisor at a number you already have, not one from the email.
We will never call or email asking for your code, a password or a payment. If someone does, don't reply. Forward it to security@wealthforms.com.
Questions
I got an email from Wealth Forms. Is it real?
Wealth Forms sends forms for financial advisors who use Altitude CRM. The email names your advisor and their firm. If you're unsure, call your advisor at a number you already have and ask whether they sent it. More for clients.
Why do I need a code to open my form?
Forms ask for personal information, so a one-time Secure Form Code is sent to you by email or text before the form opens. It expires in 10 minutes and works once. Never share it.
Do I need an account or an app?
No. Forms open in your web browser on a phone, tablet or computer.
