Got a Form From Your Advisor?
Your financial advisor uses Wealth Forms to collect and update your information securely. Here's how it works and how to stay safe.
What Is Wealth Forms?
Wealth Forms is a secure way for your advisor to send you forms, like an annual review update or a new account application. It's made by Altitude, the software your advisor uses to keep your records.
Most of the form is already filled in from what your advisor has on file. You check it, fix anything that changed and sign. It's quicker than a blank form, and safer than emailing documents back and forth.
How to Fill Out Your Form
Tap "Open your form" in the email
The link works only for you and only for that form.
Enter your Secure Form Code
We send a one-time code by email or text. Enter it on the form page. It expires in 10 minutes. If it does, tap "Send a new code."
Check each page
Confirm what's right and change what isn't. Your answers save as you type, so you can stop and come back later using the same link.
Sign and submit
You'll agree to sign electronically, then sign at the end. Your advisor reviews your answers before anything updates your records.
You don't need an account or an app. Forms work in your web browser on a phone, tablet or computer.
How to Check an Email Is Real
- It names your advisor and their firm, and you were expecting to hear from them.
- It comes from an address ending in
wealthforms.com, or from your advisor's own domain. - The form asks for a one-time code that's sent to you separately.
- Still unsure? Call your advisor at a number you already have, not one from the email, and ask.
If Something Isn't Working
My code didn't arrive
Check your spam or junk folder, then wait a minute and tap "Send a new code" on the form page.
The link says it has expired
Links expire after a while and once a form is submitted. Ask your advisor to send a new one.
I'd rather use paper
Ask your advisor for a PDF copy. When you send it back, your answers go into the same review your advisor uses for online forms.
Your Information
Your answers go to your advisor's firm. Wealth Forms stores them encrypted, only on your advisor's behalf, and doesn't use them for anything else. Read more in our privacy summary.
Who to Ask for Help
For questions about the form, your accounts or your information, contact your advisor. They can see your form and help directly. To report a suspicious email, see Report a Suspicious Email.
